I, _________________________________have given Heart of Hannah a $750.00 donation (which is tax deductible) for service fees and an additional $50.00 donation for registration and book fee.

 

I understand that if for any reason, I as a client am dismissed, or chooses to leave, monies are non-refundable.

 

This document is a receipt which will be valid for Client and Heart of Hannah. Please retain it for your records.
